碑四尺五寸、寬一尺四寸，字型工整、筆畫，是秦始皇統一文字後小篆，相傳宰相李斯所書。
刻石北、東、西三面為秦始皇詔書，共144個字，南面為秦二世元年（前209年）所刻二世詔書78個字。
刻石原立於岱頂，存29字。
清乾隆五年（1740年）遭火，刻石失；嘉慶二十年（1815年）山頂玉女池中搜得石2塊，尚存10字，移至岱廟；現在位於泰山岱廟東御座露台前西側，存二世詔書中10個殘字：斯、臣、去、疾、昧、死、臣、請、矣、臣。
清乾隆五年(1740年)碧霞祠毀於火，刻石失。
另有安國舊藏宋拓53字本，1919年三井高堅（日語：三井高堅）購得，現藏於日本三井美術館。
此刻石原分為兩部分：前半部系公元前219年秦始皇東巡泰山時刻，共144字；後半部為秦二世胡亥即位第一年(公元前209年)刻制，共78字。
刻石四面， 刻字22行，每行12字，共222字。
兩刻辭李斯所書。
現存秦二世詔書10個殘 字，即“斯臣去疾昧死臣請矣臣”，稱“泰山十字”。
據清道光八年(1828年)《 泰安縣志》載，宋政和四年(1114年)刻石岱頂玉女池上，可認讀有146字，漫滅剝蝕了76字。
明嘉 靖年間，北京許某此石移置碧霞元君宮東廡，當時存二世詔書4行29字，即“臣斯臣去疾御史夫臣昧死言臣請詔書金石刻明白矣臣昧死請”。
清乾隆五年(1740年)碧霞祠毀於火，刻石失。
嘉慶二十年(1815年)，泰安尹蔣因培帶領邑柴蘭皋山頂玉女池中搜得石2塊，尚存10個字，碑嵌於岱頂東嶽廟壁上。
清道光十二年(1832年)，東嶽廟牆坍塌，泰安知縣徐宗幹“亟索殘石於瓦礫中”，囑道人劉傳業石移到山下，嵌置岱廟碑牆內，並寫跋記其。
光緒十六年(1890年)，石盜，縣令毛大索十日，得石於城北門橋下，後重置於岱廟院內。
宣統二年(1910年)知縣俞慶瀾防刻石遭風雨剝蝕，岱廟環詠亭造石屋一所，將秦泰山刻石及徐宗乾跋和自己寫序3石嵌於石屋內，周圍加鐵柵欄保護。
1928年遷於岱廟東御座內，修築一座門式碑龕，將以上3石壘砌其中。
建國後，於碑龕正面鑲裝玻璃保護。
秦泰山刻石歷代多有摹刻拓本，現存清聶劍光摹刻拓本29字和徐宗幹摹刻舊拓本29字兩塊刻石，陳列於岱廟碑廊。
